The standard dialect of a language are usually based on a regional power, like Paris for French or Beijing for Chinese. Why is General American based on a Midwest accent?

Both Korea and Vietnam used to have a variation of the chinese writing system to write their respective languages, but both of them have since replaced it with simpler writing systems. Were there ever any attempts to do anything like that in Japan or China (outside of Mao's simplified characters)?
